Your appointment time is reserved just for you. A late cancellation or missed visit leaves a gap in the clinician’s day that could have been filled by another patient.

We require 24 hours notice for any cancellation or change to your appointment. Patients who provide less than 24 hours notice, or who miss their appointment, will be charged a cancellation fee to the card on file.

I certify that all medical and history information I provide is correct to my knowledge.

I authorize the clinic and its associated health professionals to collect my personal and medical information as documented during booking and intake. In addition, I authorize the clinic and its associated health professionals to communicate with my family doctor and referring doctor as deemed necessary for my beneficial treatment.

I understand that my personal and medical information is confidential and will only be disclosed to third parties with my permission, except as described in the privacy section below.

The Site is an e-commerce site. Our purpose is to provide at-home IV hydration, including vitamin and mineral additives, to individuals.

You also agree that services will be performed by a licensed RN, who may video and audio record the session for safety reasons, not by an advanced practice provider or MD. Nova Francis-Wallace is Medical Director for Emerald Coast IV Hydration & Wellness. You also agree that Emerald Coast IV Hydration & Wellness or its employees will not be liable for any complications, illnesses, or medical emergencies, including death, from receiving an IV treatment.

IV therapy is not a substitute for emergency care. If you are experiencing a medical emergency, call 911. Treatment is provided only after a health screening by a licensed registered nurse, and a nurse confirms availability and final pricing before your visit.

Emerald Coast Hydration reserves the right, in its sole discretion, to terminate your access to the Site and related services, or any portion of them, at any time without notice.

You agree that no joint venture, partnership, employment, or agency relationship exists between you and Emerald Coast Hydration as a result of this agreement or use of the Site. Our performance of this agreement is subject to existing laws and legal process, and nothing contained in this agreement is in derogation of our right to comply with governmental, court, and law enforcement requests relating to your use of the Site or information provided to or gathered by us.

If any part of this agreement is determined to be invalid or unenforceable under applicable law, including the warranty disclaimers and liability limitations above, then the invalid or unenforceable provision will be deemed superseded by a valid, enforceable provision that most closely matches the intent of the original provision, and the remainder of the agreement shall continue in effect.

Unless otherwise specified, this agreement constitutes the entire agreement between you and Emerald Coast Hydration with respect to the Site and supersedes all prior or contemporaneous communications and proposals, whether electronic, oral, or written. To the maximum extent permitted by law, this agreement is governed by the laws of the State of Florida, and you consent to the exclusive jurisdiction and venue of courts in Florida in all disputes arising out of or relating to the use of the Site. Use of the Site is unauthorized in any jurisdiction that does not give effect to all provisions of these Terms.

Emerald Coast Hydration reserves the right, in its sole discretion, to change the Terms under which the Site is offered. The most current version of the Terms will supersede all previous versions. We encourage you to periodically review the Terms to stay informed of updates.

We will notify you about significant changes in the way we treat personal information by sending a notice to the primary email address specified in your account, by placing a prominent notice on the website, and by updating this page. Your continued use of the website and services after such modifications constitutes your acknowledgment of the modified policy and agreement to abide by it.
